Request Form

Request form prepared in accordance with Law No. 6698 on the Protection of Personal Data

Under Law No. 6698 on the Protection of Personal Data ("Data Protection Law"), a natural person whose personal data is processed, as defined in the said Law (hereinafter "Requesting Party"), has the right to make certain requests regarding the processing of their personal data in accordance with Article 11 of the Data Protection Law.

In accordance with Article 13(1) of the Data Protection Law, our Company, as the Data Controller, requires that requests regarding these rights be submitted to our Company in writing or through other methods determined by the Personal Data Protection Board (hereinafter "Board").

In this context, requests submitted "in writing" to our Company shall be made by printing this form and:

By personal submission by the Requesting Party,
Through a notary public,
By signing with the "secure electronic signature" as defined in Law No. 5070 on Electronic Signature and sending it to our Company's registered electronic mail address,

shall be submitted to our Company.

Your requests submitted to us will be answered within thirty days from the date your request reaches us, in accordance with Article 13(2) of the Data Protection Law, depending on the nature of your request.

This request form has been prepared to identify your relationship with our Company, to completely identify your personal data, if any, that is processed by our Company, and to enable us to answer your request correctly and within the applicable legal timeframe. Our Company reserves the right to request additional documents and information to identify and verify your authority in order to prevent legal risks arising from unlawful and unjust data sharing and especially to ensure the security of your personal data.
